A new program has launched at LHSC to ease the transition from the clinical setting to a more familiar one. It’s called ‘LHSC to Home.’
Those eligible can receive at-home services for up to 16 weeks. It’s targeted at long-term patients, such as those with more complex needs.
Elizabeth Hutchison had been one of 500 who participated in the initiative this year. She experienced a range of medical issues in 2025. These included COVID-19, a leg infection, and an ulcer.
“The ability for me to stay at home was a lot to do with the fact that they (LHSC) were flexible. […] They will do what you need, and that has to be different for every patient.”

“They have the opportunity to sleep in their own bed, to be with their family, maybe their pets. They get to turn the light on and off when they choose to,” said Vice President of Patient Flow, Tammy Quigley. She has noticed that being in a familiar environment makes recovery quicker and more effective overall.
Quigley also pointed out that the initiative will help free up more hospital beds. LHSC stated that more than 500 people have already been assisted with the initiative this year.
SE Health have partnered with LHSC to deliver the services. The organization is a not-for-profit enterprise.
Interdisciplinary teams are assembled to develop each patient’s individualized care plan. This includes PSWs, physiotherapists, and doctors.
The initiative is being supported by the Ontario Ministry of Health. Hutchison hopes that it becomes the norm.
